I'm pretty sure it's by Susan Gott, Phoenix Art Glass Studio, Tampa, Florida.

You are absolutely right David & Fossilfly - it is Susan Gott - so the mystery is solved!
I just found few very similar blocks browsing google images.
Actually she has a website where you can see quite few of this kind.
They seem to be sand cast and very ornate. Take a look here for future reference:
